Apply to Serve on the Supervisory Committee

Thank you for your interest in OnPoint’s Supervisory Committee.

The Supervisory Committee is responsible for ensuring that the Board of Directors and management of the credit union:

  1. Meet required financial reporting objectives through the comprehensive annual audit.
  2. Establish practices and procedures sufficient to safeguard members’ assets.

Applications are currently being considered for the 2019 committee appointments. The reference materials listed at right are updated annually in September.

To be considered for the role of Supervisory Committee member, please complete the steps below.

    Prior to completing the online Supervisor Application, please review each attachment, particularly the Supervisor Position Description. Links to each reference can be found to the right. **Please note the reference materials are updated annually in September.

  1. Complete the Supervisor Application
    Upon completing the online application, you will be asked to attach your resume detailing your employment history (Employer; City, State, County; Position; Dates Employed; and Reason for Leaving), as well as other relevant professional activities and affiliations.

  • Part I - Supervisor Agreement to Serve
  • , do hereby request that my name be considered by the Supervisor Appointment Committee for a position as Supervisor of OnPoint Community Credit Union for the 2019 appointment, to be made at the April 2019 meeting of the Board of Directors.

    I understand that in order to be appointed, I:
    1. Must have current primary membership preferably for twelve consecutive months before the annual meeting (March 14, 2019).
    2. Must not be a current employee of the credit union, or not have been employed by the credit union within the past three years.
    3. Agree that I may not serve on the Board of Directors and the Supervisory Committee at the same time.
    4. Must have no convictions of a felony or greater charge, or any criminal charge involving dishonesty or a breach of trust.
    5. Must be bondable.
    6. Must sign the Agreement to Serve and Letter of Resignation

  • I certify that, if appointed, I am able to serve without any conflict of interest, and agree to comply with established code of conduct requirements.

  • I agree to the placing of my name in consideration and will serve, if appointed as a Supervisor, to the best of my ability.

  • I agree to complete the CUNA Volunteer Achievement Program (VAP) Credit Union Fundamentals modules and Supervisory Committee I and II self-study programs with self-administered competency tests, offered through the Northwest Credit Union Association, within two years, and to complete orientation, annual online compliance training, and other professional development requirements as the committee may establish from time to time.
